A signed consent form is only worth anything if you can produce it later, and prove what the client actually agreed to. Paper gets filed and lost. A PDF emailed back doesn't say when it was signed or by whom. And when you update your wording, last year's signatures quietly start referring to a form that no longer exists.

Every completed form is stored with the date and time it was signed, the signer's name, and the IP address and device it was signed from.
The wording is frozen at the moment of signing
Wellspring keeps an unchangeable copy of the form as it stood when the client signed it, so changing your template later does not change what an earlier client agreed to.
Consent that expires, and consent given by someone else
Set a validity period per form and Wellspring flags the clients due to sign again. If a parent, guardian or carer signs on the client's behalf, their name is recorded alongside the signature.
Signed forms can be opened and printed from the client's record whenever you need to produce one.

What happens when we update our consent wording?
New clients sign the new version. Everyone who signed the old one keeps a copy of the old one, word for word as it was on the day.
Can a parent or carer sign for a client?
Yes — the form records the name of the person who signed alongside the signature itself.
